Billing

FFC Billing and Service Policy

Effective: 1/13/2025 

 At Fernbrook Family Center (FFC), we are dedicated to providing clear, supportive financial policies to ensure our services remain accessible to everyone. Below, you’ll find details on our policies and the various payment options available to accommodate your needs 

Insurance Billing and Payment Responsibilities

  • Insurance Billing:  With your consent, we will bill your insurance company for any services provided to you or your child.
  • Credit Card on File: Clients with private insurance and those paying out of pocket are required to keep a credit card on file.
    • Please don't hesitate to reach out if you don’t have a credit or debit card to put on file. We are happy to discuss alternative arrangements to accommodate your needs.
  • Invoices and Payments:
    • You will receive a monthly invoice for charges not covered by your insurance.
    • Full payment is due within 30 days. If payment is not received, the credit card on file will be charged.
    • For automatic payments, contact the FFC Business Office at 507-446-0431. Payments are processed twice a month (on the 1st and 15th).
  • Financial Assistance: If you are unable to pay the full balance:

  1. Visit www.fernbrook.org to complete the credit card authorization form.
  2. Contact the Business Office to:
    • Set up a payment plan.
    • Apply for a sliding fee scale.

         3. Discuss service frequency with your provider.                                                            

  • Unpaid Balances:
    • Balances unpaid for 120 days will be referred to a collection agency.

Appointment Scheduling and Cancellation

  • Cancellations:
    • Notify us at least 24 hours in advance if you need to cancel an appointment.
    • For private pay or non-Medicaid insurance, a fee of $18 may apply for late cancellations or no-shows.
  • Missed Appointments:
    • Services may be discontinued after three consecutive no-shows (missed appointments without notice).
    • More than four last-minute cancellations (less than 24 hours’ notice) may also result in paused or withdrawn services.
  • Unresponsive Clients:
    • If we are unable to contact you for two weeks (after attempts via phone and mail), your file will be closed.

Insurance Eligibility

  • Medicaid Clients:
    • Eligibility is checked monthly. If your insurance is marked as “ineligible,” services may be paused until insurance is reactivated or another payment source is identified.
