CLIENT PORTAL

  

Fernbrook Family Center has implemented a Client Portal option for all clients and caregivers. The portal will allow clients/caregivers to sign paperwork and view their clinical record. 


To register for a login, please provide your email address to your provider. After receiving your email address, you will receive an email from Procentive. Procentive is the electronic medical record that is utilized by Fernbrook Family Center and is a safe and HIPAA compliant platform. Please activate your account right away as the link does expire. 


Already have a login? You can access the portal here 

At this time we do not have an app version of the client portal. You must access the portal through a web browser.
