Client Portal

Fernbrook Family Center has introduced a Client Portal option for all clients and caregivers. This portal enables clients and caregivers to sign paperwork and access their clinical records securely.


To register for a login, please provide your email address to your provider. Once your email address is received, you will get an email from Procentive. Procentive is the electronic medical record system used by Fernbrook Family Center and is a safe, HIPAA compliant platform. Make sure to activate your account promptly, as the link will expire.


Already have a login? You can access the portal here.


Currently, we do not offer an app version of the client portal. You must access the portal through a web browser.
