Children's & Adult Targeted Mental Health Case Management (TCM)
Targeted Case Management helps children, adolescents, and adults with serious mental illness access the services and supports they need to achieve their goals. Case managers work collaboratively with clients, families, healthcare providers, schools, and community partners to coordinate care, navigate resources, and promote long-term stability and well-being.
Assessment
A case manager will meet with you and your family to assess the needs of the client and their families. This allows us to create a list and prioritize what will be the most helpful to start with.
Planning
Together, the case manager and the client will create an individualized support plan that is centered around goals designed to help clients flourish in their relationships and in the community.
Referral & Linkage
The Case Managers will assist you in referring and linking the you and your family to necessary resources to help you achieve your goals.
Monitoring & Coordinating
Case Managers will provide you with stepping stones, breaking down barriers as they assist in coordinating services for you and your family. The Case Manager will monitor progress and make adjustments for success as needed.
