POSITIONS AVAILABLE IN GOODHUE COUNTY

Adolescent Day Treatment Therapist

We are currently looking for an IMPACT Adolescent Day Treatment Therapist in our Goodhue County location. In this role you will provide individual and group therapy, conduct diagnostic assessments to children ranging from 10-18 years old.


The IMPACT Adolescent Program is designed for adolescents who need more support than traditional outpatient therapy services provides, incorporating mental health supportive skill building and psychotherapy in a controlled structured setting.

Qualifications:

  • Master's degree in one of the behavioral sciences (psychology, social work, counseling, and/or related field)
  • Must be licensed or working towards licensure in one of the following license types: LICSW, LPCC, LMFT, or LP
  • Have an understanding of the importance of adolescent and childhood development into young adulthood.
  • Excellent communication, advocacy and collaboration skills preferred
  • Must have reliable transportation, a valid driver's license, valid automobile insurance, and acceptable driving record
  • Must pass criminal background check based on agency and Department of Human Services requirements


Responsibilities:

  • Provide therapy services to children and potentially their families in center based, outpatient, school, and in home/community settings
  • Complete any required paperwork for therapy cases (DA, CASII, psychotherapy treatment plans)
  • Provide support to the Adolescent Day Treatment center
  • Attend weekly/monthly meetings
  • Promote teamwork and effective communication within Fernbrook
  • Attend any day treatment or relevant program meetings including curriculum tracking and building meetings
  • Continue to build and maintain relationships with the community (i.e. attending IEP meetings, completing observations for school per request, collaborating with external and extra disciplinary providers, supporting treatment team staff in working with the clients we serve)
  • Maintain progress towards licensure and follow requirements of supervision as applicable
  • Coordinate with clinical supervisors and review admittance procedures for potential clients into day treatment programs
  • Other duties as assigned

Case Management Assistant/Intern

 

The Case Management Assistant/Intern, will be responsible in assisting Case Management Team in areas of intake and assessment which determine eligibility for targeted management services.

This position is part-time, 16-24 hours/week.

Qualifications:

  • Applicants must be enrolled in a bachelor’s degree program related to Social Work, Psychology, Clinical Psychology, and other pertinent related fields
  • Excellent communication, advocacy and collaboration skills preferred
  • Must have at least 30 semester hours/ 45 quarter hours relevant coursework
  • Must have reliable transportation, a valid driver’s license, valid automobile insurance, and acceptable driving record
  • Must pass criminal background check based on agency and Department of Human Services requirements


Responsibilities:

  • Complete mental health targeted case management activities for children, adults, and families as they relate to intake and assessment.
  • Utilize various effective strategies to respond to child/adult/family needs in a respectful and professional manner.
  • Complete all documentation according to content and quality standards.
  • Assure clients meet the case management eligibility requirements, including: diagnostic assessment, insurance, follow up with families about timelines, other needs as determined.
  • Maintain contact and assist clients during the intake and assessment process.
  • Provide case management services beyond intake and assessment, as identified.
  • Meet with the child/adult/family at a frequency based on client mental health need and be available to meet with the child/adult/family upon their request, during intake and assessment process.
  • Complete intake and initial paperwork, including: intake paperwork, eligibility and notification forms, IFCSP, etc.
  • Coordinate and facilitate transfer from intake to ongoing case management provider.
  • Link referred clients with services if they are determined ineligible for case management services.
  • Communicate and reflect Fernbrook Family Center’s mission internally and externally.
  • Comply with agency policies and procedures.

Qualifications:

Candidates must qualify as an In-Home Behavioral Therapist/Mental Health Practitioner (MHP) in one of the following ways:

  • 4000 hours of experience in the delivery of services to adults or children with mental health illness
  • Has a bachelor’s degree in a behavioral sciences or related field and has completed an internship/practicum in the delivery of services to adults or children with mental health illness
  • 30 semester hours/45 quarter hours (about 1 year as a full-time student) relevant coursework and 2,000 hours of experience in the delivery of services to adults or children with mental health illness
  • Masters or other graduate degree in a behavioral science or related field


Responsibilities:

  • Helping clients manage their mental health symptoms by providing skills based training to youth, adults, families, and groups
  • Ability to work in a group setting or working with client(s) individually 1:1
  • Ability to preserve professionalism when working with client(s) and their families
  • Maintain all required documentation
  • Non-traditional work hours including some evenings
  • Some travel is required
